EMPLOYMENT Application
FPB will not accept a resume in place of an application. However, you may attach your resume to the completed application, which must be signed.

Applications are available at the Frankfort office of the Department for Employment and Training located at 1121 Louisville Road, Pine Hill Plaza, Suite 6, Frankfort. The application can also be downloaded as an Adobe PDF document on this page.

Once downloaded, the application can be printed for completion. You may mail the printed application to:

REMEMBER, be sure to complete and sign the application.
